THE SITUATION ON SITE

A permit-required confined space entry is one of the few work activities where a single procedural gap can be fatal within minutes. Oxygen-deficient or toxic atmospheres, engulfment hazards, and mechanical hazards inside tanks, vessels, pits, sewers and process equipment mean entry has to be controlled every time, without exception.

The barrier at the point of entry is the last physical line of defense before the permit system: a manhole cover left open, an unmarked hatch, or an access point without a physical barrier invites an unauthorized entry, a dropped tool into the space below, or a fall into an opening that should have been secured.
