THE SITUATION ON SITE
Deck hatches, floor gratings, manholes and inspection openings get removed for access during maintenance and often stay open longer than planned – between shifts, over a lunch break, or while a crew moves on to the next task. An open hatch with no cover and no barrier is a fall hazard for anyone crossing that area.
Below the opening, work often continues: a confined space entry, machinery, or a walkway used by other crews. A tool, part or piece of debris dropped through an open hatch can seriously injure whoever is underneath, with no warning.
